Before an event

• Audiogram data sheet - completed by an audiologist no later than three months before the event
• Parental/guardian agreement - required for under-18 players and includes supervision, conduct, medical and media consent
• Online tournament and supporter registration

After or during an event

• Complaint form - concerns about conduct, access, safety, facilities or organisation
• Feedback form - rate the event and suggest improvements
• Refund request - provide booking, payment and supporting details

Event operations

• Player attendance and sign-in record
• Risk assessment and control measures
• Incident and near-miss report
